Eligibility Requirements
Pre-Participation Physical Examination
All students must complete a pre-participation physical examination before participating in any athletic activity, including a tryout. A typical packet will consist of 8 pages, but a 9th page will be added for students with special needs. You must use our packet for a physical to be accepted and all pages must be included. Make sure each page is completed with all requested information and signatures. Forms completed by medical professionals must clearly specify the date of the exam.
Completed packets are only used within a school year. Only exams performed after May 1 may be used in the next school year. For example, if a student turned in a physical in March for soccer tryouts, that paperwork will not be used in the next school year. If that student wanted to tryout for softball the following August, they would need a new physical. This requirement has been imposed so that we can be in compliance with paperwork filing mandates.
Physicals do not need to be performed by a student's primary care physician. They may be completed at an urgent care facility, a CVS Minute Clinic, or a nurse practitioner; to name a few options. Prices for some of the alternatives range from $25 to $35. Print out the complete packet from the menu on the left, complete your portion, and take the entire packet with you.
To turn the completed packet in, staple the documents together and insert them in the "Athletics Drop Box" in the cafeteria or hand them to the receptionist at the front desk. These are the only acceptable ways to submit your physical. Do not hand them to your coach or teacher.

Academic Eligibility per SCCPSS
ELIGILBILTY: In order to be eligible for any athletic activity, the athlete:
Must meet all eligibility requirements prior to the first tryout/practice date.
Must not participate (practice or play) if ineligible.
Transfer students and new students must have transcripts on file before an athlete is eligible to participate.
If a child is retained for any reason, he/she is considered ineligible for athletics during that school year.
Must pass 3 out of 4 academic subjects (Math, Science, ELA, Social Studies) in the semester immediately prior to competition.
Must have cumulative semester connections average above 70 in the semester immediately prior to competition.
May only participate once as a 6th grader (not eligible for football), once as a 7th grader and once as an 8th grader, not to exceed 3 years (2 years for football) regardless of the numbers of schools attended.
Failure in more than one (1) academic subject (Math, Science, ELA, Social Studies) during a given 9 week grading period shall declare a student to be ineligible for practice and competition for the remainder of the semester.
Must not participate (practice or play) in any athletic event if assigned to In-School Suspension (ISS) or Out-Of-School-Suspension (OSS) during the assigned time. Students returning from suspension may face further participation limitation as a result of missed practice, lack of conditioning, etc. (See Student Conduct).
Must not have reached his/her 16th birthday prior to May 1 of their 8th grade year.
A player must not reach his/her 15th birthday prior to May 1 or their 7th grade year.
A player must not reach his/her 14th birthday prior to May 1 of their 6th grade year.
PRACTICE
Must complete a SCCPSS athletic participation form and physical examination and turn in to the school’s athletic director. All football players must have the football helmet warning form. The physical portion of the form is valid only for 365 days from the date of examination.
All students who participate in athletics shall have an annual physical examination prior to participation in any tryout, practice or conditioning, whichever comes first. The physical examination form shall indicate approval for participation and be signed by a doctor of medicine or doctor of osteopathy
A player injured requiring medical attention may not participate in practice or a contest without a doctor’s note. A student absent from athletic practice 5 or more days due to illness or injury shall receive a medical release from a licensed physician before returning to practice or play.
A player must practice a total of six (6) days before playing in a game in all sports except football, where a player must practice in ten (10) days.
Must not participate in activities, practice and/or games on days they are absent from school.
Must be present 50% of the student day on the day of an athletic contest in order to participate in the event.
Must not, as an individual or a team, practice or play during the school day.
Must not play, practice, or assemble as a team with your coach on Saturdays, Sunday, holidays or vacation days or if school closes early.
STUDENT CONDUCT
In order for a student to participate in athletics he/she must follow the Student Code of Conduct concerning conduct grades. Students assigned out-of-school- suspension will result in a loss of eligibility. Coaches, Athletic Director, and School Administrators reserve the right to revoke student eligibility due to student conduct.
Overall Needs Improvement (N) Conduct Grade Consequences:
Students receiving a needs improvement conduct grade may be referred to a behavior modification plan with identified behaviors and specific consequences.
Overall Unsatisfactory (U) Conduct Grade Consequences will be:
Be applied immediately,
Carry through the next marking period (consequences for unsatisfactory conduct grades earned in the fourth marking period will carry over through the first marking period of the following school year).
Any personal recognitions or serving as a representative of the school will be denied, including recognition for academic achievement.
Participation in any extra-curricular activity will be forfeited (e.g., sports, cheerleading, clubs, events, band, chorus, etc.).
